Lumen Health
AI triage for primary care clinics
Seed Round ยท $2.5M
May 2026 ยท Confidential
Investor One-Pager

Cutting primary care wait times by 73% with AI triage that doctors actually trust.

Lumen embeds in the EHR, auto-prioritizes patient messages, and frees up 9 hours of clinician time per week โ€” saving the average clinic $87k/yr in burnout-driven turnover.

โš  The Problem

Clinic inboxes are drowning.

A solo PCP gets 60โ€“120 patient messages a day. Triaging them eats 2.5 hours of clinical time per provider, drives 78% of burnout-related quits, and pushes urgent cases behind requests for refills.

โœ“ Our Solution

An AI triage layer that fits in 15 minutes.

Lumen reads inbound patient messages, classifies urgency, drafts a clinician response, and surfaces only the messages that need a human. Sits inside Epic, Athena, eClinicalWorks โ€” no new login, no workflow change.

How to use this investor one-pager template

VCs read about 200 decks per week and remember none of them. They remember one-pagers. A clean, dense, well-structured one-pager lives on a partner's iPad and gets passed across the table during a Monday partner meeting. A 30-slide deck dies in the inbox. This template gives you the structure used by Series-A pitches that closed in 2024-2026 โ€” the problem statement that lands in 30 seconds, the traction chart that lets a partner see growth at a glance, the financial ask phrased as an outcome instead of a beg.

Who this is for

Pre-seed and seed-stage founders, accelerator applicants, indie hackers raising friends-and-family rounds, and anyone who needs to pitch their company in a single document.

The 7 sections of a working one-pager

1. Headline + tagline. Company name. One sentence describing what you do, who for, and why it matters. "We help [audience] [solve problem] by [unique mechanism]." Cut every adjective.

2. The problem. What's broken. Quantified. "73% of solo therapists lose 4 hours per week to scheduling friction." Real numbers, real sources.

3. The solution. What you built. Three bullets. Each bullet is a feature paired with the user outcome.

4. Traction. The number that proves it's working. MRR, paying customers, signed LOIs, monthly active, retention curve โ€” whichever number is most flattering. If you have a chart, use it. If you don't have one, build a clean table.

5. Market. TAM, SAM, SOM. Honest numbers. Investors are good at spotting inflated TAMs. If your real serviceable market is $400M, say so โ€” and explain how a $50M outcome is plausible for a great team.

6. Team. Two-three founders max. Each one a one-line credential ("Stanford CS, ex-Stripe payments lead, previously sold X for $42M to Y"). Why YOU are the team that wins this.

7. The ask. Round size, what you'll do with the money, time horizon. "Raising $1.5M seed to ship product to 10 paying enterprise customers and hit $50K MRR by Q3 2027."

Step-by-step: how to use this template

1. Open the sample. Click each placeholder text and replace with your own.

2. Don't expand sections. The constraint of "fits on one page" is what makes it work. If you can't fit your story in 7 short blocks, you don't know your story yet.

3. Test the 8-second read. Show it to a friend for exactly 8 seconds, take it back, ask them what your company does. If they can't tell you, the headline isn't tight enough.

4. Get one specific number above the fold. "300 paying customers" or "$80K MRR" or "growing 30% MoM." Numbers anchor everything else.

5. Print it. Send it as PDF. Investors print things they take seriously.

Tips that matter

Don't lead with the team. Lead with the problem. Investors invest in painful problems. The team matters but the team is the answer to "can YOU solve this?" The problem is the question.

Pick a single traction number and make it the hero. Three weak numbers feel weaker than one strong one.

Update the one-pager monthly during a raise. Stale numbers are a red flag. A line at the bottom like "Numbers as of [last month's end]" shows you're current.

Skip the long competitive matrix. Investors don't have time to read a feature comparison. One sentence: "Closest competitor is X; we win on [specific edge]."
